Subscription

1.  Print the subscription form or order it from HealthInvest Partners.

2.  Send the subscription form to HealthInvest Partners, either by fax on                 +46 8 440 3839 or by mail to the address below. 

3.  Deposit the payment on the Fund’s account with SEB:                                  Bank account number: 5565-10 377 80

4.  A contract note will be issued to you after the end of the month.

Redemption

1.  Print the redemption form or order it from HealthInvest Partners.

2.  Send the completed form to HealthInvest Partners, either by fax on                +46 8 440 3830 or by mail to the address below. The request must have reached HealthInvest Partners not later than five banking days prior to the end of the month.

3.  A contract note will be issued in conjunction with disbursement of the redemption amount to the unit holders account.
